Perrigo initiated a voluntary recall of two batches of its generic children’s cough syrup sold in 4 oz. bottles — guaifenesin grape liquid and guaifenesin DM cherry liquid — after discovering that its dosage cups had incorrect markings, which led to fears of an overdose risk. The FDA urges consumers who bought these batches to “discard the dosing device and product.” CNN (1/13)

The products have been sold at CVS and Rite-Aid among others. See affected products and lot numbers here.
